Introduction: Bariatric Surgery (BS) has contributed to mitigating the impacts ofobesity on comorbidities and cardiometabolic risk factors. This investigation aims toevaluate the remission of diabetes mellitus (DM2) and the reduction of cardiometabolicrisk, of obese patients with DM2, after five years of BS and followed up in the PublicHealthcare System (PHS). Methods: This is an observational, retrospective, andanalytical study carried out in 71 volunteers, with pre-diabetes or DM2, with five yearsor more of conventional treatment (CT) or submitted to BS, being 33 from the BS group38 of the CT. Socioeconomic, lifestyle, anthropometric, biochemical, medication,cardiovascular, and glycemic parameters were evaluated. Results: The baselinecharacteristics of the BS and CT samples were similar, being predominantly women(91.6%), with a mean age of 46.1 ± 9.0 years. BS group patients presented an averagewaiting time of 28.3 ± 24.4 months for the BS, and 93.9% were submitted to the gastricbypass surgical technique. Comparing the BS group with the CT, BS patients had ahigher educational degree (p=0.001), a higher prevalence of social alcohol drinkers(p=0.006), and a greater degree of obesity (p<0.001). After 5 years, 22 (95.7%) and 20(74.1%) of the BS group patients had total remission of DM2 and cardiometabolic risk,respectively (p<0.0001), counterpart, 1 (4,3 %) and 7 (25.9%) of the patients in the CTgroup had remission of DM2 and cardiometabolic risk, concomitantly. The ratio ofDM2's persistence chances was lower in the BS group, the intermediate cardiovascularrisk (OR:1,27; IC:1,07- 1,50; p <0,008), the excess weight loss (OR:1,00; IC:1,00-1,01;p < 0,0001) also had higher ratios as chances of happening in the BC group.Conclusion: BS is probably higher than CT in promoting the remission of pre-diabetes/DM2, the reduction of cardiometabolic risk and the number of medicines used,and the improvement of biochemical and anthropometric markers of obese patients.
